Financial Policy Information
In-Network Plans:
We are in-network with the following insurance providers:
- Delta
- Delta Medicare Advantage
- Cigna PPO
- United Concordia
Out-of-Network-Plans:
Most patients with out-of-network insurance plans still get coverage in our office. We are happy to submit claims on your behalf. While we will assist in this process, please note that your insurance provider determines your coverage, and you are ultimately responsible for any estimated or remaining balance.
We understand that navigating insurance benefits can be challenging. If you’d like an estimate of your out-of-pocket costs before your appointment, please contact our office. We are happy to provide a detailed estimate to help you plan accordingly.
Payment Due at Time of Service:
To help our office run efficiently and ensure timely care for all patients, we ask that payment be made at the time of service. For patients with dental insurance, this includes your estimated copayment or deductible.
Post-Insurance Balances:
Once your insurance claim is processed, any remaining balance will be your responsibility. We will send you a statement for any outstanding amount, and prompt payment is appreciated.
Estimates:
Before beginning treatment, we will provide you with a good faith estimate of your expected costs based on your specific treatment plan and insurance coverage. Please understand that this is an estimate, and final costs may vary based on the specific treatment provided, which could change during the course of clinical treatment based on varying circumstances, and your insurance company's determination of benefits.
Payment is expected at the time of service unless other arrangements are made in advance. For your convenience, we accept the following forms of payment:
- Cash
- Personal checks
- Credit and debit cards (Visa, Mastercard, Discover, and American Express)
- Health Savings Accounts (HSAs) and Flexible Spending Accounts (FSAs)
- CareCredit financing
To help make your care more accessible, Elm Endodontics offers CareCredit financing. CareCredit provides flexible payment plans, including interest-free options for qualified applicants. If you’d like to learn more or apply for CareCredit, please contact our office or visit CareCredit.com.
We understand that unexpected events happen. If you need to cancel or reschedule your appointment, we ask for at least 24 hours’ notice. Missed appointments without prior notice may result in a missed appointment fee.
Your time is valuable, and so is ours. By notifying us in advance, we can better accommodate patients who may need urgent care.
If you are experiencing discomfort or pain, we will do our best to see you promptly. Emergency appointments after hours may require additional fees depending on the complexity of care provided. Please contact our office if you have concerns about an urgent dental need.
We know financial matters can sometimes be confusing, and we are here to help. If you have any questions about your insurance coverage, payment options, or our financial policy, please contact our office via voice or text at 303-427-2769, email us at info@elmendo.com, or use the contact form.
We are committed to making your experience at Elm Endodontics as seamless as possible, both in delivering your care and in handling your financial needs.
Common Financial Questions
We understand that dental care can be a significant investment. We offer CareCredit to help patients manage larger balances with flexible payment plans. Please contact our team to discuss your options.
We strive to provide clear information upfront, but sometimes changes are made in the course of treatment that will affect cost estimates. We'll do our best to provide a detailed estimate based on your treatment plan and insurance coverage.
If your insurance company denies a claim, we will work with you to understand the reason and discuss your options. While we assist in this process, the final responsibility for payment rests with the patient.
Depending on the complexity of your treatment plan, we may request a deposit to secure your appointment time. Our team will discuss this with you during scheduling.
If you were told this, what the person meant to say was that we are “not in network with your insurance.” What that means is that we have no prior arrangement with your insurance company about a designated fee structure.
Even if we are not “in-network” with your insurance, you will usually still get coverage, but it will be on a more flexible fee schedule. Call our office so that we can help you navigate your plan and get an estimate for services we provide.
